URL is malformed,only one number follow % and t is the last parameter.
eg http://test.com/test.fcgi?p1=test&p2=%E1%9

Thanks for the bug reports. They've both been committed the the repository.

On a more curious note, in what situation did you find that the ++i==size ==> ++i>=size actually led to problems? I was trying to figure it out but I couldn't see a situation where it would happen.
