It seems that if there is a transformation set with a nonzero vertical offset when the first glyph is loaded using the autohinter, from then on the autohinter will produce characters with inconsistent heights. Attached image shows the incorrect behavior on top, and correct on bottom, using Windows' Arial font.
Reproduce with the following code fragment:

An easy workaround is to ensure that the first glyph loaded with the autohinter has a null offset.
