lsof shows opened files for each thread, when you have 6 threads running in xmms, you have six entries in lsof, although the file is opened only once.

(Sound Blaster PCI 512, ALSA 0.9.0rc7)

When I'm playing something in XMMS using this plugin, my box will freeze and I get this high-pitched whine coming from my speakers. All my other sound apps work fine, including UT2k3 (The ultimate stress test :), so I got suspicious of some sort of foul play with XMMS. So I booted it up, started playing, and ran "lsof | grep /dev/snd". I nearly fell off my chair:

xmms 7703 roothorick mem CHR 116,16 408 /dev/snd/pcmC0D0p
xmms 7703 roothorick 9u CHR 116,0 392 /dev/snd/controlC0
xmms 7703 roothorick 11u CHR 116,0 392 /dev/snd/controlC0
xmms 7703 roothorick 12u CHR 116,16 408 /dev/snd/pcmC0D0p
xmms 7704 roothorick mem CHR 116,16 408 /dev/snd/pcmC0D0p

xmms 7712 roothorick mem CHR 116,16 408 /dev/snd/pcmC0D0p
xmms 7712 roothorick 9u CHR 116,0 392 /dev/snd/controlC0

xmms 7712 roothorick 12u CHR 116,16 408 /dev/snd/pcmC0D0p

This, honest to God, came from ONE session of XMMS. I have a suspicion this is the cause of my crashes...
