Re. the comment about Outlook users not being able to see attachments: that might be due to a bug in some version(s) of Outlook when there's a Content-Id in the message. See the mhbuild man page for how to disable this using -nocontentid.

Done, I just committed support for Content-Disposition in mhbuild directives. It uses a new, optional {} construct. See the mhbuild man page for syntax and example.

Yes! Damn mhbuild makes nearly everything look like Outlook's insiteful default ATT*.att However I think a best stop-gap
would be to add the name of the attached file as the disposition.
As for extended syntax a 3rd arg for build to provide the name seems reasonable.

But what I'd really like to see is a new version!

I agree. This prevents me from using nmh to mail attachments to Outlook users as they often reply to me that there is nothing attached to my mail. If nothing else, a simple fix would be to repeat the Content-description line, adding an attribute of inline. Then you don't have to devise a new syntax for Content-disposition in mhbuild's language.
